Fibromyalgia Is More Than Pain

When most people think of fibromyalgia, they think of chronic pain. While pain is one of the most recognizable symptoms, it is only one part of a much larger picture. Fibromyalgia is now understood to involve changes in nervous system function, affecting the way the brain and nervous system process pain and other sensory information. These changes can make the nervous system more sensitive, causing everyday sensations to feel more intense and making it difficult for the body to "turn down the volume" on pain. Researchers refer to this as central sensitization, and it is considered one of the key processes underlying fibromyalgia (Clauw, 2014).

These changes in nervous system function help explain why fibromyalgia affects nearly every aspect of daily life—not just the muscles or joints. Many people experience overwhelming fatigue, sleep that never feels restorative, difficulty concentrating ("fibro fog"), headaches, digestive problems, dizziness, and heightened sensitivity to light, sound, touch, temperature, or even certain smells. For many individuals, these symptoms can be just as disruptive as the pain itself, making it harder to work, care for family, maintain relationships, or enjoy the activities they once loved.

The encouraging news is that our understanding of fibromyalgia has advanced tremendously over the past two decades. Brain imaging studies and other research have shown measurable differences in how the nervous system processes pain and sensory information, helping confirm that fibromyalgia is a real, biologically based medical condition—not something that is "all in your head" (Clauw, 2014; Häuser et al., 2015). This growing body of research has also changed how experts recommend treating fibromyalgia. Rather than focusing on pain alone, today's evidence supports a comprehensive approach that addresses sleep, stress, physical activity, emotional well-being, and changes in nervous system function (Macfarlane et al., 2017).

Why Fibromyalgia Affects the Entire Body

One of the most common misconceptions about fibromyalgia is that it is simply a disorder of the muscles or joints. In reality, fibromyalgia is now understood to be a disorder of the central nervous system—the brain and spinal cord—which help regulate how the body processes pain, fatigue, sleep, and other sensory information. Over the past two decades, advances in neuroscience have shown that people with fibromyalgia often experience central sensitization, a condition in which the nervous system becomes more responsive to both painful and non-painful stimuli. This heightened sensitivity can amplify ordinary sensations and helps explain why fibromyalgia affects so much more than pain. Research has also identified changes in brain regions involved in pain processing, alterations in neurotransmitters that regulate pain signals, disruptions in restorative sleep, and abnormalities in the body's stress-response system, all of which may contribute to the wide range of symptoms experienced by people living with fibromyalgia (Clauw, 2014; Häuser et al., 2015; Macfarlane et al., 2017).

Understanding fibromyalgia through the lens of nervous system function—not simply pain—changes the way we think about fibromyalgiatreatment. Because multiple body systems are involved, the most effective care goes beyond medication alone. Current clinical guidelines recommend a comprehensive, multidisciplinary approach that combines medical care with evidence-based behavioral interventions, physical activity, sleep optimization, and strategies that improve the nervous system's ability to regulate stress and pain.

Widespread Pain

Widespread pain is the hallmark symptom of fibromyalgia, but it is unlike the pain most people have experienced after an injury or overuse. Rather than being confined to one area of the body, fibromyalgia pain is often felt on both sides of the body and above and below the waist. It may be described as aching, burning, throbbing, stabbing, or a deep soreness that seems to move from one area to another. Some days the pain is mild and manageable, while other days—even simple activities like getting dressed, climbing stairs, or carrying groceries can feel overwhelming. Many people also notice that their pain becomes worse during periods of stress, poor sleep, illness, weather changes, or after doing too much physical activity, often leading to what is commonly called a fibromyalgia flare.

Current research suggests that this pain is related to changes in nervous system function, rather than ongoing injury to the muscles or joints. Studies have shown that people with fibromyalgia process pain differently than individuals without the condition. The brain and spinal cord become more responsive to pain signals—a process known as central sensitization—causing sensations that might normally be uncomfortable to feel significantly more painful (Clauw, 2014). Researchers have also identified differences in the activity of brain regions involved in pain processing, as well as changes in neurotransmitters that help regulate how pain signals are amplified or dampened (Häuser et al., 2015). These findings help explain why pain can persist even when medical imaging or laboratory tests appear normal.

Although fibromyalgia pain can feel relentless, it is important to remember that it is manageable. Effective treatment focuses not only on reducing pain intensity but also on improving the way the nervous system regulates pain over time. International treatment guidelines support a comprehensive approach that combines education, regular physical activity, healthy sleep habits, stress management, and evidence-based psychological interventions such as cognitive behavioral therapy (CBT) as core components of care (Macfarlane et al., 2017).

Fatigue That Sleep Doesn't Fix

For many people living with fibromyalgia, fatigue can be even more disabling than the pain itself. This isn't the kind of tiredness that improves after a good night's sleep or a quiet weekend. Instead, it is a deep, persistent exhaustion that can make even routine tasks feel overwhelming. You may wake up after eight or nine hours of sleep feeling as though you barely slept at all. Activities that once required little effort—preparing a meal, running errands, concentrating during a meeting, or spending time with family—may leave you feeling completely drained. Many patients describe feeling as though their "battery never fully recharges," regardless of how much they rest.

Research suggests that this fatigue is related to changes in nervous system function rather than a lack of motivation or physical conditioning. People with fibromyalgia commonly experience disruptions in restorative, deep sleep, even if they spend enough time in bed. Studies have found abnormalities in sleep architecture, including reduced slow-wave sleep and frequent nighttime awakenings, which interfere with the body's ability to restore energy and recover from the demands of the day (Moldofsky, 2001; Clauw, 2014). At the same time, changes in pain processing, heightened nervous system sensitivity, and the body's stress-response systems can create a cycle in which pain disrupts sleep, poor sleep worsens fatigue, and fatigue increases the body's sensitivity to pain. Breaking this cycle is an important part of successful fibromyalgia treatment.

Although there is no single solution for fibromyalgia-related fatigue, there are effective, evidence-based strategies that can help. Current treatment guidelines recommend addressing sleep quality, physical activity, stress management, and pacing of daily activities rather than relying on rest alone (Macfarlane et al., 2017).

Fibro Fog

Many people living with fibromyalgia describe one symptom as particularly frustrating: "fibro fog." While it is not an official medical diagnosis, the term is widely used to describe the cognitive difficulties that commonly accompany fibromyalgia. You may find yourself forgetting appointments, struggling to recall familiar words, losing your train of thought during conversations, or feeling as though it takes much more effort to concentrate or make decisions. Reading a book, following a complex conversation, or multitasking at work may suddenly feel far more difficult than they once did. These experiences can be unsettling, especially for individuals who have always relied on their memory and mental sharpness. Fortunately, fibro fog is a well-recognized symptom of fibromyalgia and does not mean you are developing dementia or permanently losing your cognitive abilities.

Research suggests that fibro fog is related to changes in nervous system function rather than structural damage to the brain. Studies have shown that people with fibromyalgia often perform more slowly on tasks involving attention, working memory, and information processing, particularly when they are experiencing higher levels of pain or fatigue (Glass, 2009; Clauw, 2014). Brain imaging research also suggests that the brain may be working harder to complete everyday cognitive tasks because it is simultaneously processing persistent pain signals. In other words, when the nervous system is constantly managing pain, fewer cognitive resources may be available for concentration, memory, and problem-solving. Poor sleep, chronic fatigue, stress, anxiety, and depression can further contribute to these cognitive difficulties, making fibro fog more noticeable during periods of symptom flare.

The encouraging news is that fibro fog is often manageable. While there is no medication specifically designed to treat cognitive symptoms in fibromyalgia, research suggests that improving sleep quality, increasing physical activity at an appropriate pace, managing stress, and reducing overall symptom burden can lead to meaningful improvements in cognitive functioning (Macfarlane et al., 2017).

Sensory Sensitivity

Many people are surprised to learn that fibromyalgia can make the body more sensitive to much more than pain. You may find that bright lights seem painfully intense, everyday sounds feel overwhelming, certain fabrics become uncomfortable against your skin, or strong scents trigger headaches or nausea. Some people become unusually sensitive to changes in temperature, while others notice discomfort from a gentle hug, a seatbelt across the shoulder, or clothing that never bothered them before. These experiences can be confusing and frustrating, especially when family members or friends do not understand why seemingly ordinary sensations can feel so overwhelming.

Research suggests that these sensory sensitivities are related to changes in nervous system function rather than problems with the eyes, ears, or skin themselves. The nervous system plays an important role in filtering and interpreting the countless sensory signals we encounter every day. In fibromyalgia, that filtering process appears to become less efficient, causing the brain to respond more strongly to sensations that would normally be ignored or perceived as only mildly uncomfortable. This phenomenon is closely related to central sensitization, which helps explain why many people with fibromyalgia experience increased sensitivity to touch, sound, light, temperature, and other sensory input in addition to chronic pain (Clauw, 2014; Häuser et al., 2015). Brain imaging studies have further demonstrated that people with fibromyalgia process sensory information differently than individuals without the condition, supporting the understanding that these symptoms have a measurable biological basis (Schmidt-Wilcke & Diers, 2017).

Although sensory sensitivity can make daily life more challenging, it is not something you simply have to "live with." Learning to recognize your personal triggers, improving sleep quality, managing stress, pacing activities, and using evidence-based strategies to regulate the nervous system can often reduce the intensity of these symptoms over time.

Digestive Symptoms & Irritable Bowel Syndrome (IBS)

Digestive symptoms are surprisingly common in people living with fibromyalgia. Many individuals experience frequent abdominal pain, bloating, constipation, diarrhea, nausea, or a feeling that their digestive system is unpredictable. Some are also diagnosed with Irritable Bowel Syndrome (IBS), one of the most common conditions that occurs alongside fibromyalgia. These symptoms can be just as disruptive as chronic pain, making it difficult to enjoy meals, travel, attend social events, or feel confident leaving home. If you have noticed that your digestive symptoms seem to worsen during periods of increased pain, stress, or fatigue, you are not alone.

Research suggests that the connection between fibromyalgia and IBS is related to changes in nervous system function rather than a problem with the digestive tract itself. Both conditions are considered disorders of altered pain processing, meaning the brain and nervous system become more sensitive to signals coming from the body. In IBS, the digestive tract may become overly responsive to normal stretching and movement, causing symptoms even when no structural disease is present. Researchers also believe that the close communication between the brain and the digestive system—often called the brain-gut axis—plays an important role. Changes in this communication can influence pain perception, bowel function, and sensitivity to stress, helping explain why fibromyalgia and IBS frequently occur together (Clauw, 2014; Häuser et al., 2015; Aziz et al., 2016).

Headaches & Migraines

Headaches and migraines are among the most common symptoms experienced by people with fibromyalgia. Some individuals develop frequent tension-type headaches, while others experience debilitating migraines that may include throbbing pain, nausea, dizziness, and increased sensitivity to light, sound, or smells. These symptoms can make it difficult to work, spend time with family, or participate in everyday activities. Many people also notice that their headaches become more frequent during periods of poor sleep, increased stress, or fibromyalgia flare-ups. If this sounds familiar, it is important to know that headaches and migraines are recognized as common conditions that often occur alongside fibromyalgia—they are not simply a coincidence.

Research suggests that the overlap between fibromyalgia and migraines is related to changes in nervous system function. Both conditions involve central sensitization, meaning the brain and spinal cord become more responsive to pain and other sensory signals. As a result, the nervous system may amplify sensations that would normally be less noticeable, contributing to widespread pain, headaches, and increased sensitivity to light, sound, and touch (Clauw, 2014; Häuser et al., 2015). Studies have also shown that migraines occur significantly more often in people with fibromyalgia than in the general population, suggesting that the two conditions share common biological mechanisms involving pain processing and sensory regulation (Marcus et al., 2005; de Tommaso et al., 2017).

Mood Changes: Anxiety & Depression

Living with fibromyalgia can affect much more than your physical health—it can also have an impact on your emotional well-being. Many people with fibromyalgia experience symptoms of anxiety, depression, or both at some point during their lives. It is understandable why this happens. Coping with chronic pain, unrelenting fatigue, poor sleep, and the uncertainty of symptom flare-ups can be emotionally exhausting. At the same time, research suggests that mood changes are not simply a reaction to living with chronic pain. Fibromyalgia and mood disorders appear to share some of the same biological pathways, meaning the connection is more complex than many people realize.

Research suggests that changes in nervous system function contribute to both the physical and emotional symptoms of fibromyalgia. The same brain regions and neurotransmitters involved in processing pain also play important roles in regulating mood, stress, and emotions. Chronic pain can place the nervous system in a persistent state of heightened alertness, making it more difficult to manage stress and increasing vulnerability to anxiety and depression (Clauw, 2014; Häuser et al., 2015). Studies have also shown that people with fibromyalgia are more likely than the general population to experience anxiety and depressive disorders, underscoring the importance of addressing emotional health as part of comprehensive fibromyalgia care (Arnold et al., 2016; Galvez-Sánchez & Reyes del Paso, 2020).
